TOURIST MT VISA (MEDICAL TREATMENT)

Attention : For Medical visas scheme (Tourist MT or Non Immigrant – O), the applicants are strictly advised to contact the Royal Thai Embassy/Consulate General for authorization prior to their e-Visa application submission. Any unauthorized application submission under Medical visas scheme would be rejected and non-refundable.

Visa and Certificate of Entry for Non-Thai nationals who need to receive medical treatment in Thailand (except for treatment for COVID-19), including their accompanying persons (no more than 3 accompanying immediate family members)

Step 1:
Applicant contacts hospital in Thailand that has been approved by the Ministry of Public Health (MOPH) to receive foreign patients and quarantine foreign patient and to send a request to the Department of Health Service Support (HSS), along with (1.1) Confirmation Letter (1.2) Affidavit of Support (1.3) Summary Form of Vehicles for Patients and other related documents such as a passport.

Step 2:
If approved, HSS will send the list of approved patients and accompanying family members to the Ministry of Foreign Affairs to inform the Embassy to issue a Certificate of Entry (COE) and visa.

Step 3:
Completed the Online Visa Application (OVA) at least 10 working days before the intended date of travel

Required Documents:

1. Visa Application form(s) and photograph(s): Completed the Online Visa Application (http://visaonline.thaiembassy.sg/ ) , sign Visa Application form as well as bring recent photographs (size 3.5×4.5 cm). Applicants are required to present:

        •  2 application forms and 2 photographs: China (for Tourist Visa/Transit Visa applicants Only), Nigeria

        •  3 application forms and 3 photographs: Afghanistan, Algeria, Bangladesh, Cameroon, China (for Non-Immigrant Visa applicants Only), Central Africa Republic, Congo, Congo DRC, Egypt, Equatorial Guinea, Ghana, Guinea, India, Iraq, Lebanon, Liberia, Libya, Nepal, North Korea, Pakistan, Palestine, Sao Tome and Principe, Saudi Arabia, Sierra Leone, Sri Lanka, -Sudan, Syria, and Yemen.

        •  4 application forms and 4 photographs: Iran

2. Passport (valid for no less than 6 months)

3. Copy of Confirmation Letter with the stamp from MOPH:(Letter of certification from the hospital in Thailand confirming the admission of the patient and arrangement of quarantine within the hospital for the period of at least 14 days (for the patient and their attendants))

4. Evidence of adequate finance to cover stay in Thailand (such as bank statement which states bank contact information and deposit)

5. Copy of Affidavit of Support Letter with the stamp from MOPH (for accompanying persons)

6. Required document for a Certificate of Entry (COE) application (submission online is required)

        •  A copy of health insurance covering all expenditures of medical treatment, including
          COVID-19 for the entire period of stay Thailand in an amount of at least 100,000 USD

        •  A copy of Alternative Hospital Quarantine (AHQ) confirmation

        •  A copy of a valid ticket with Singapore Airlines

7. Singapore’s Long-term pass

If applicable, a copy of the applicant’s long-term pass (e.g. Singapore Blue IC and Re-Entry Permit Form 7 (for Singapore’s Permanent Residents), Employment Pass, Personalised Employment Pass, S Pass, Work Permit, Entry pass, Training Employment Pass, Student Pass, Dependent Pass, Long Term Social Visit Pass, or Professional Visit Pass). The passes must also be valid for at least two months. If the pass validity is less than 2 months, a letter of explanation from the guarantor (e.g. employer/spouse’s employer) is required.

8. Application fee (Cash only in Singapore Dollars – Non-refundable):

9. Other documents: as may be requested by Consular officers in addition to the documents above. Please note that Consular officers reserve the right to require additional documents, or an interview with the applicant, as deemed necessary, without prior notice.
